RE: Recommendations for Draft CY 2027 Medicare Payment Policies Under the Physician Fee Schedule (PFS)

Dear Dr. Oz:

On behalf of the undersigned 16 leading patient advocacy and professional societies we submit the following suggestions as you prepare a proposed rule updating the MedicarePhysician Fee Schedule (the Proposed Rule) for calendar year 2027. We appreciate the agency’s focus on wellness, prevention and better management of chronic diseases. We are hopeful that you will revisit your request for specific examples of Medicare codes that don’t sufficiently address services for effective management of chronic diseases like osteoporosis.

Last year, we provided you with recommendations focused on the prevention of costly repeat fractures caused by poor management of osteoporosis, a highly prevalent and costly chronic disease. We urge you to include our recommendations in your proposal for calendar year 2027 as their implementation would reduce unnecessary Medicare chronic disease costs and fulfill the mandate in President Trump’s February 13th 2025 Executive Order, ‘‘Establishing the President’s Make America Healthy Again Commission”. As you know, this order calls on CMS and other health agencies to ensure the effective management of chronic diseases and to “ensure the availability of expanded treatment options and the flexibility for health insurance coverage to provide benefits that support beneficial lifestyle changes and disease prevention.”
